Grace Hill Neighborhood Health Centers, (GHNHC), a Federally Qualified Health Center, was established in 1906. Grace Hill provides professionally directed primary and preventive health care through five community health centers serving low income and uninsured residents in 45 census tracts in the City of St. Louis and St. Louis County.

Grace Hill’s Integrated Health and Support Services Model includes, but is not limited to, the following services:

Primary Health Care is provided by skilled physicians and nurse practitioners on staff at each site. Services include pediatrics, adult medicine, and OB/GYN services.

In addition, Grace Hill provides dental services, optometry, pharmacy, transportation, medical social services, x ray, mammography, WIC (Women, Infants and Children program), and Division of Family Services on site eligibility verification and enrollment services.

Community Health and other services involve prenatal and pediatric case management provided by nurses and nurse assistants to high risk pregnant mothers and babies. Through Grace Hill’s Health Coaching model, neighbors are trained to help other neighbors access health services. Mobile outreach teams provide health education and referrals through three Health-To-Go-Vans, and Grace Hill’s Children's Developmental Center serves children under 12 years of age with developmental and behavior problems.

Grace Hill participates in several National Health Initiatives:

Diabetes Collaborative, Smoking Cessation, Asthma, Cardiovascular Disease, Redesign of the Patient Visit, Lead Poisoning Prevention, Advanced Open Access to improve patient access to appointments and reduce the no-show rate, and BCCP/Cancer.

Grace Hill coordinates two city wide health initiatives to reach special populations - the Homeless and Public Housing residents. Grace Hill’s community health nurses serve almost 30 homeless shelters, and services include Respite Care for patients recuperating from serious illnesses, as well as on-site dental services provided by Grace Hill’s Mobile Dental Team. Primary care for public housing residents include targeted services for school children living in subsidized housing.

Grace Hill was accredited by JCAHO in February 2004. JCAHO accreditation means Grace Hill meets the highest standards for ambulatory health care centers
