Nurse Case Management

The nurse case management department provides case management services to prenatal clients, high risk infants, children with asthma, children with lead poisoning, children with immunization delay, families who over use the emergency room as well as other families referred to the department by the medical staff.  In the year 2001 nurse/nurse assistant teams case managed 692 prenatal clients.  These clients received 1,792 home visits during their pregnancies.  These teams also provided case management services to 517 high-risk infants, 425 children with asthma, 276 children with lead poisoning, and 222 families with inappropriate use of the emergency room as well as approximately 250 other children with various health problems or risk factors.  These children received a total of 2680 home visits in 2001.  A state of the art computer tracking system supports all case management activities.  This system assists the case managers in monitoring the clients’ care plan and in making sure that the clients receive the necessary services.  Grace Hill Neighborhood Health Center’s receives reimbursement for case management services from Care Partners, Health Care USA and Missouri Medicaid.

The nurse case managers and nurse assistants also conducted 396 prenatal classes in the health centers with 732 participants.  They participated in five health fairs providing health information and linkage to 440 participants.  They also conducted 17 blood pressure screening sessions in the community screening 283 clients.

In-Home Services

The In-Home Services Department provides homemaker/chore and personal care services to eligible elderly and handicapped residents of St. Louis City. These services assist clients with personal care and help to improve and maintain a safe sanitary environment so that clients can remain in their homes rather than having to be moved to a nursing home.  The program also provides nurse set-up of medications as well as needed nurse assessment of client progress and status.  This program is funded through reimbursement from the Missouri Department of Health and Senior Services. In 2001 the department provided 27,604 units of homemaker/chore/personal care services to 45 clients.  The department also made 402-nurse home visits to 12 clients.

WIC (Women, Infants, and Children)

Grace Hill Neighborhood Health Centers offers WIC services at five of the six health centers and also at Barnes Jewish Hospital.  This program provides supplemental infant formula and other nutritional food supplements to pregnant women and children less than six years of age.  The program also offers nutritional counseling, education and assessments to the target population.  The WIC Program also provides support and education to clients who choose to breastfeed their infants.  In 2001 the WIC Program provided services to approximately 2,400 clients per month.  The total WIC encounters for the year equaled 28,910. 

Head Start

Grace Hill Neighborhood Health Centers began providing health services to Head Start children in August of 2001.  A Health Services Manager, a Disabilities Coordinator, a Registered Dietician, two Community Health Nurses, and four Health Care Coaches are responsible for the health care needs of the Head Start children.  They coordinate medical and dental care and screening of the children as well as the health and nutrition education of teachers, parents and children. The nurses and Health Care Coaches provided height and weight and hearing and vision screening on all of the children. During the past six months the health center played a major role in meeting the Head Start health performance standards by providing physical exams and dental screenings on children who were delinquent in these areas.   The staff assumed responsibility for safety and safety equipment for the teachers and the 700+ children currently enrolled in Head Start.  All of the relevant Head Start staff has been taught CPR and First Aid.  They also made sure that First Aid kits and safety posters are posted in all classrooms.
